Call your preferred office or use the appointment link. Then download our new patient forms to fill out before your visit.

Arrive 10 min early
Bring your ID, insurance (if any), and completed forms.

Comprehensive exam
We take digital X-rays, do a full oral exam, and discuss your history.
- Step 4
Clear plan
We explain what you need, when, and how much it costs. You decide.
What to bring
- ✓Completed patient forms (download them on our Patient Forms page)
- ✓Photo ID
- ✓Dental insurance card (if you have one)
- ✓List of current medications
- ✓Any prior X-rays you have
Payment and financing
We accept all PPO dental plans, cash, cards, CareCredit and Cherry at all four offices, and our team can review other third-party financing options with you. We discuss payments without pressure.
